Recommendations for the management of neurosurgical pathologies in crisis period linked to COVVI-19
by the French SOCIETE of Neurosurgery (SFNC)

The current health crisis calls for a profound change in the capacities of reception and surgical management of patients. Phese imperatives are essential: limiting the risks of interindividual contamination, especially during hospitalization; limit the risks for operated patients (mortality of covid+patients); Limit the need to resort to resuscitation or continuous care units. In the event of necessity, patients can be transferred to a competent center nearby and less saturated. However, the need for emergency care and care that cannot be delayed given the loss of luck that this constitutes.
Common sense is essential in the decision. Each subsection of the company will make it due to some targeted recommendations, but this information is only decision supports for each team depending on the situations of the moment and the guidelines of the guardianship. For example, the following situations seem relevant:

1) Maintaining surgical management in emergency

  • Serious head trauma
  • Spontaneous hemorrhagic pathologies
  • Infectious pathologies and infectious complications of an intervention.
  • Dysfunctions of an LCS derivation system or intrathecal dissemination.

2) Surgical management to be maintained in the context of “organized” activities

  • Malignant tumors (high -grade gliomas, metastases and other fast -growing malignant tumors), including by radiochuruse;
  • Benign cranial and spinal tumors only in case of htic or functional deficit of rapid or brutal evolution (visual disturbances, hemiplegia, paraplegia, etc.);
  • Filling of intrathecal diffusion pumps. Change of stimulators (e.g. dystonia)

3) All the other care must be different, on a date which cannot be specified at present and which will depend on the evolution of this health crisis. See the article on the SNFC Méningiomes and Covid 19 website (DE SFNC recommendations), which can have masks for free?
